Missing information from your import is usually caused by your CSV headers on row 1 being incorrect. If the header is edited, incomplete, or not compatible with the system you are importing to, errors often occur.

To correct this issue, verify that the header information, Row 1, on your CSV file matches the column title names on row 1 in the spreadsheet linked above. It is important the names are not edited, re-arranged, or removed entirely as doing so will cause the import to either fail or import incorrectly.
